   Diploma Request Cardis used by the Registrar's Office for four purposes: to enter your name for an anticipated degree date; to provide an audit of your academic program information; to allow you to indicate special instructions for mailing and imprinting your diploma; and to insure your receipt of Commencement information. After you complete your Diploma Request Card, please submit it to the Executive Education Programs Office for signatures and filing with the Graduate School.    Electronic Access to Maxwell To obtain access to the Complex as well as the appropriate secured computer lab, 040 Eggers (204A is secured by combination lock), using your SUID, please come to the office of Executive Education Programs. This form is NOT available on the internet.
 
 
  Certificate of Full Time StatusSyracuse University considers you a full-time student, and the Registrar can confirm your full-time status directly, if you are matriculated in a graduate degree program and meet other specific criteria.  Complete this form and submit it to the Executive Education Programs Office for filing.
 
 
  Program of Study This form when signed by your department is used by the Graduate School to certify your degree completion.  After you complete your Program of Study, please submit it to the Executive Education Programs Office for signatures and filing with the Graduate School. The downloadable form is located here , in our office or visit the Registrar or Graduate School. Click here or check the Academic & Event Calendar for the date your Program of Study must be filed to qualify for a specific degree conferral date.
 
 
   Petition to FacultyAt anytime you are requesting special consideration to the Department's, Graduate School's or University's academic policies/deadlines, you will use this form. After obtaining the required signatures, submit it to the office of Executive Education, after retaining a copy, we will forward it to the Registrar's office for consideration. 
 
 
  Request for IncompleteStudents who are unable to complete a course within normal time limits because of exceptional circumstances may be eligible to request an Incomplete grade.  Until the time that the Incomplete grade is removed or after the time specified for completion, the Incomplete is calculated as a F in your GPA. 
 
 
  Exec Educ Update FormPlease use this online form to change or update your information/grant permission for its use by Executive Education Programs in various places.  This form is separate and distinct from the process of updating your records with the University - you may do that online with the Registrar.
 
 
  Add/Drop These forms are used to make schedule adjustments after the SCORE registration deadlines.  Full tuition will be charged for courses dropped after the financial drop deadlines.  Add/Drop forms are available in the the Executive Education Program office. You are required to fill in the highlighted places on the form. Be sure to get the signature of the instructor, the Public Administration Department and Catherine Gerard. It is NOT necessary to have the signature of the college dean.
